Camden Local Care Strategy: delivering care closer to home and supporting people to care for themselves.
Camden CCG is holding a live Twitter chat on Friday 24th June, 12.30pm - 13.30pm, run from the account @camden_ccg, with the hashtag #CamdenLocalCare. The chat is a way of hearing and responding to Camden residents' feedback and questions, and to inform our priorities and decision making as we move forward in implementing Camden's 'Local Care Strategy' (which you can download below).
We want to answer your questions and hear your views on what works well, what doesn't work well and what changes you would make to the services you receive from local hospitals, GPs and community care. Our expert panel, Lance Saker (GP at Hampstead Group Practice, Clinical Vice Chair and Governing Body Clinical Lead for Long Term Conditions, Camden CCG) and Sally MacKinnon (Transformation Director, Camden CCG) will be poised and ready to answer your questions and respond to your feedback.
We will start the chat by asking Camden residents and stakeholders the following questions:
1. Joined up care – what does good joined up care look like for patients/service users?
2. Access – how can we make the system easier to understand and navigate?
3. Self-care – how can we empower people to be able to care for themselves more?
